Description:
We are seeking a detail-oriented Nursing - Population Health Data Intern to join our Population Health Clinical team.
In this role, you will help analyze patient data and clinical/medical information to create actionable insights for our healthcare partners, supporting their efforts to provide proactive, value-based care.
Intus Care is a rapidly growing healthcare technology company focused on improving care delivery for complex, older adult populations.
Our platform helps healthcare organizations deliver better outcomes through data analytics, care coordination, and population health management.
Key responsibilities include reviewing and analyzing population health data using the Intus Care analytics platform to identify high-risk patients and trends.
You will review patient medical records to identify clinical trends and outcomes to inform population health initiatives.
You will create clinical reports summarizing individual patient profiles, population segments, and care strategies providing insights into clinical outcomes and potential strategies to drive improvements.
You will create and distribute weekly clinical intelligence reports highlighting patients with increasing risk scores, recent incidents, or concerning utilization patterns.
Collaboration with clinical team members to understand care patterns and develop meaningful insights is essential.
You will support healthcare partners in utilizing data analytics to improve care delivery and patient outcomes.
Maintaining strict confidentiality of protected health information (PHI) is a critical responsibility.
Requirements:
You must be currently pursuing a bachelor's or master’s degree in the nursing field.
Strong analytical and critical thinking skills are required for this position.
Excellent written and verbal communication abilities are necessary.
Proficiency with data analysis and Microsoft Office Suite is essential.
You must have the ability to maintain strict confidentiality and handle sensitive information appropriately.
Preferred qualifications include experience with healthcare data analytics or population health management.
Familiarity with value-based care models is advantageous.
Knowledge of HIPAA compliance requirements is preferred.
An understanding of geriatric care and complex care management is beneficial.
